What hospital-to-home looks like with us.
The first few days at home after hospital are the riskiest. Our hospital-to-home service starts on the ward — coordinating with the discharge team, picking up equipment and making sure the home is set up safely before you arrive.
In the first two weeks we provide more frequent reablement visits — focused on regaining confidence, mobility and routine — then taper back to longer-term support if it's still needed.
We work with your GP, district nurse and therapy team so nothing falls between services.
