About this service

What end of life care looks like with us.

End-of-life care is the most important work we do. Our team supports clients to die at home, on their terms, with the people and routines they love around them — working alongside hospice, GP and district-nurse teams.

We provide day visits, overnight cover and full live-in care during this stage, with extra carer support so no one is exhausted.

Care plans include the practical (symptom management, mouth care, pressure-area care) and the personal (music, photographs, faith, family rituals).
