What living with diabetes looks like with us.

Diabetes care at home is about the small daily routines that keep things stable — and the quick response when they're not. Our team supports blood-glucose monitoring, insulin administration, foot checks and meal planning, with a clear escalation path to the GP and diabetes nurse.

Where carers administer insulin, they hold an in-date competency sign-off and follow a written protocol agreed with the GP.

We're particularly experienced with older clients managing diabetes alongside other long-term conditions.

    We agree monitoring times, insulin doses and meal timings with you and the diabetes nurse.

    Trained team

    All carers on the rota are competency-signed for insulin admin and hypo response.

    Daily checks

    Glucose, foot and skin checks logged digitally and reviewed weekly.

  4. 04

    Escalation

    Out-of-range readings trigger an immediate call to the coordinator and, if needed, the GP.

Can carers give insulin?

Yes, where they have a current competency sign-off and a GP-approved care plan.

What happens in a hypo?

Carers follow a written hypo protocol, give fast-acting glucose, and call 999 or the GP per the plan.

Do you support CGM users?

Yes — we work with Libre and Dexcom monitoring as part of the daily routine.
